Metropolitan Polyclinic specialist warns of tuberculosis diagnosis

Among the 20 medical specialties available at the Policlínica Metropolitana, in Belém, the Pulmonology Service has an installed capacity to serve up to 700 people each month. The specialty is responsible for tracking airway pathologies and diseases that affect the respiratory system. Although it is not the unit’s profile, specialists from the Metropolitan Poli warn of care with the disease. With a wide range of exams, the pathology can be identified in the Diagnostic Center itself. Treatment continues in Primary Health Care.

According to estimates by the World Health Organization (WHO), one third of the world’s population is infected with Mycobacterium tuberculosisand at risk of developing the disease. There are about 8.8 million sick people and 1.1 million deaths per year on the planet.

Metropolitan Polyclinic offers diagnostic tests for the diseaseBrazil ranks 17th among the 22 countries responsible for 82% of tuberculosis cases worldwide. Although it is a disease that can be prevented, treated and cured, it still kills around 4,700 people every year in Brazil.

Pulmonary tuberculosis is transmitted when a person coughs, the droplets of the disease bacillus are in the air and another person comes in and breathes. That is why in closed places – such as prisons and nursing homes – there is greater transmission.

Prevention – The coordinator of the Pulmonology Service of the Polyclinic, Flávia Farinha Ayres, explains that, for prevention, “it is necessary to have control of chronic diseases that can lead to a drop in the body’s immunity, and to carry out a screening assessment of contacts, in the case of living with a newly diagnosed person”.

The doctor advises the patient with suspicion of the disease to seek a health center, where the doctor will indicate the relevant tests. In case of confirmation of the diagnosis, treatment and follow-up will be provided at the Basic Health Unit itself. “In the basic regimen, four medications are used in the first two months, and in the other four months we keep only two. They are: Isoniazid, Pyrazinamide, Rifampicin and Ethambutol, which must be maintained for six months in the usual treatment”, adds Flávia Ayres.

The pulmonologist also clarifies that the Polyclinic is not a reference for the treatment of tuberculosis, which must be carried out in Basic Units (UBs). In cases where there is diagnostic doubt, management of important adverse effects or special situations is necessary. Patients should be referred to the Tuberculosis Secondary Referral Service at Hospital João de Barros Barreto.

Specialty – The Polyclinic is a unit with a service for specialized medical care, where the evaluated patients have chronic respiratory symptoms. “The most common are coughing and dyspnea. After the investigation, confirmed the diagnostic hypothesis, we institute the treatment and follow up until the stabilization of the condition, when we must return the patient to the basic network for follow-up, and he can be referred to the specialist for reassessments, depending on the need “, details the pulmonologist.

According to the coordinator of the Service, care in the specialty is offered from Monday to Friday, but there may be changes, depending on the doctor’s schedule. “The vacancies are all previously informed to the State Regulation for scheduling, after directing the clinical doctor, in UBS or in the Polyclinic itself, or directed by another medical specialist who verifies the need for the evaluation of the pulmonologist during his consultation”, informs Flávia Ayres.

The coordinator of the Infectious Diseases Service of the Polyclinic, Lorena Martins, points out that the unit offers the population sputum tests, chest X-rays and tomography of the chest. “We are able to diagnose tuberculosis. However, we started the treatment and sent him to the health center. Generally, pulmonary tuberculosis stays in the basic unit, and when it arrives at the Poli, it is like lymphatic, ocular and cerebral”, informs Lorena Martins.

calls – The Executive Director of the Policlínica Metropolitana, Liliam Gomes, emphasizes that the attendance at the Pulmonology Service at the Metropolitan Poli is offered by State Regulation, guaranteeing the observance of the Unified Health System (SUS). The patient must go to the Basic Health Unit and, with a medical request, will be referred to the Polyclinic by the State Regulation System (Sisreg). Another way to be attended in the specialty is with internal referral.
